A Clear, Private Intake Process
Complete the intake form.
Provide accurate contact, service, household, and needs information.
Allow time for review.
A submission is a request, not a guarantee of assistance.
Respond to follow-up.
PSO may need verification, documents, or additional details.
Receive available support or referral.
PSO will explain the next step based on current resources.

Personal information should be collected only for service coordination and communication. Sensitive documents should use a secure intake system.
